Medical homes can lead to higher quality and lower costs, and can improve patients’ and providers’ experience of care.
NCQA Patient-Centered Medical Home (PCMH) Recognition is the most widely-used way to transform primary care practices into medical homes.
We will be sending reminders to patients for some of the following services if you are due for them, Mammograms, PSA Blood tests, Blood Pressure follow up, Pulmonary Function Tests, Diabetic Screening tests, Immunizations and other health maintenance services.

He has been certified in the past with NCQA DRP and PCMH which recognize clinicians who deliver consistent high evidence-based quality care to diabetic and primary care patients.

If you are admitted to area hospital or facility a provider/hospitalist will be assigned to you during your stay.
Please ask that ALL of your hospital records are faxed to Dr. Sofat upon your discharge.
Also, schedule a follow up within 1-2 weeks of your discharge, and bring discharge paperwork and all of your medications with you to your follow up appointment after hospitalization.
